Car Park/Issues
The underlying issue with this car park is that while it appears the driver is at fault by over spilling into the neighboring bay, there is a fault with how the car park has been designed. From what I can see, it appears that there is dead space to the left of where he is parked, which looks as if it cannot accommodate for another car anyway due to the arc, which I can only assume is a driveway for navigating around the car park. On the other hand the driver may have noticed this flaw and realised he is at an advantage being parked in that bay. Alternatively he may have a large item that needs to be loaded into the back seat of his car and it may have not been possible to gain access to the back door with the space that he is given.
Changing the design
There several ways in which this design could be rectified. One example would be to re-paint the carpark bays into a grid system. That way you would not have any arcs or curves which would interrupt the flow of the bays. Another method would be to prevent drivers from using the dead space by using obstructive methods. This could be done by making use of the yellow buffers which are already been used to prevent drivers from vertically overlapping the bays. Additionally you could plant a tree or put a sign in the area of the dead space to prevent drivers from using it and gaining an advantage of extra parking space. A more subtle approach would be to use hatch markings.

Hospital image problems
The issue with the system implemented in this hospital is user interpretation. The image shows what I assume to be a dividing line, to ensure people keep to the left of the corridor, or the right depending on the country. In this case it is the left as I can see a couple keeping to their left and walking away and a gentleman keeping to his left walking towards. What makes the image interesting is that there is another gentleman walking on the dividing line down the centre of the corridor. It is easy to assume that he is been lazy and just wants to walk down the middle, however it could that the yellow line is actually a waypoint to a certain location. This is a yellow line, and there could be waypoints of other colours leading to different segments of the hospital. So the issue here is that it does not state explicitly if this is a way point or a dividing line to maintain the flow of traffic.
